When I launch ISim from Project Navigator or via a simulation executable, a fatal error occurs after the ISim splash window appears. The error states that there has been an application error and the software must close. Details of the error point to problems with the "libqt_gui.dll" library.
How can I resolve this error?

This is a known issue. It has been fixed in ISE Design Suite 11 Update 3 (11.3).Please download and install the latest ISE Design Suite update from the Download Center at: http://www.xilinx.com/support/download/index.htm.
